About the Role

As a Care Manager, you will deliver high-quality, comprehensive care to customers within the Home Care Packages Programme. You will utilise your specialised clinical expertise to ensure effective support at home, and develop individualised care plans that address complex needs. You will manage daily administrative tasks, support program operations, and provide regular reports to the Program Manager on key performance indicators (KPIs) and budgetary performance.

Responsibilities include:

  • Leads the support and supervision activities related to Home Care Packages within the Packaged Care Program, coordinates with multiple service providers to meet customers' needs, and adheres to legal requirements, ensuring compliance in all aspects of care.
  • Serves as a subject matter expert in their clinical specialty, conducts comprehensive clinical assessments and develops care plans to deliver high-level care to customers at home.
  • Manages daily administrative duties efficiently, supporting program operations and reports to the Program Manager on key performance indicators (KPIs) and budgetary performance.
  • Oversees the development and implementation of innovative health programs tailored to the needs of customers and Special Needs Groups. Manages care coordination and complex cases proficiently to ensure optimal customer outcomes.

About You

To be successful in this role, our preferred candidate will have:

  • Bachelor’s Degree required; Aged Care, Community Services, Social Science, Nursing or Allied Health is preferred. Any other related discipline or commensurate work experience considered.
  • Significant experience in delivering health services or community-based services, particularly in the Aged Care industry is preferred.
  • Registered Nurse with Australian Health Practitioner Regulation Agency (AHRPA), Cardiopulmonary Resuscitation (CPR) Certification, Current Senior First Aid Certificate is preferred.
  • Strong knowledge of Patient Case Management, Clinical Operations, Clinical Practices and Standards, and Healthcare Management Systems is preferred.
  • Excellent Talent Management, Cross-Functional Team Working, Effective Communication and Customer Experience skills.
